Intake Coordinator – Support at Home

Full Time | Help at Home Team | Belair + Field-Based

Kalyra is seeking a friendly, organised, and community-focused Intake Coordinator to help older South Australians access the support they need. This role is perfect for someone who enjoys meeting people, building relationships, and creating a smooth onboarding experience for new clients.

Job Description

About the Role

  • Be the first point of contact for new clients and referral partners

  • Respond to enquiries and conduct in-home sign-ups

  • Complete intake documentation and initial risk screening

  • Coordinate a smooth handover to our care and clinical teams

  • Build relationships with GPs, hospitals, assessors, and community groups

  • Attend occasional community events to promote our services

  • Maintain accurate records and support reporting across AlayaCare/CRM

Desired Skills and Experience

What We’re Looking For

Essential:

  • Strong customer service skills

  • Confident communicator; able to explain fees and services clearly

  • Highly organised with strong attention to detail

  • Valid Driver’s Licence

  • Comfortable with MS365 & care management systems

  • Able to meet screening requirements (WWCC/NDIS/Police Check)

Desirable:

  • Experience in Aged Care (SaH, HCP, CHSP, or private)

  • Understanding of My Aged Care and ACAT/RAS pathways

  • Knowledge of South Australian community networks
